The Team
Kelly Lorenz
Owner
What has driven me, from my early days as a yoga instructor, then founder & owner FORM 01945, was the quest to build a collective space for the health and wellness of our clients, founded on a strong sense of community. I hold the passionate belief, as both a fitness instructor and a mother, that a fitness studio is a place to shape the wellness of body, as well as that of the mind and spirit. STRENGTH IS A MINDSET As a mother of two, a wife and a daughter, I appreciate that life challenges often get thrown at us out of the blue. In 2021, my beloved father was diagnosed with terminal cancer and passed away six weeks later. What I discovered during this painful experience was how key our fitness community was to the process of healing from grief, not only for myself, but for many of our members, who come here to strengthen their bodies and increase emotional well-being through the shared belief that we can, together, achieve anything. With the mantra borrowed from inspirational guru Glennon Doyle,”You can do hard things,” our clients are encouraged to take agency over their health and wellness, both physically and spiritually. I take pride in cheering on my clients; I know that it’s not easy for every individual to suit up, show up and leave everything on the mat. Those victories should be celebrated.
Peri West
GENERAL MANAGER / MEMBERSHIP COORDINATOR / TREAD / FORM / PERSONAL TRAINER
15+ years as a head NCAA Div III collegiate lacrosse coach. Started 2 programs from the ground up; Mount Ida College in Newton, MA and Hendrix College in Conway, AR. I have international coaching experience as the head coach of the first Colombian National Team and coached them during the 2017 World Cup. I hold a masters degree in Sports Management with a concentration in coaching from Lasell University.
I played collegiate lacrosse at Wheaton College, was a 2x captain and 3x NEWMAC first team All-conference player. While at Wheaton I received the “Caroline Cutler” female leadership award and was a 3 year member and president of the Student Athlete Advisory Committee.

I love teaching at FORM because of the people and the style of workouts. The upbeat music, the ever changing workouts and the motivation I get from the instructors is what keeps me coming back. I love the feeling of accomplishment I get after these workouts and I know you will too!
Anne Moore

19 years of experience in the fitness industry including personal training, small group training, group exercise and CrossFit coaching. Big believer in dialing in technique, using compound movements & tailoring a client’s program to what THEY need/want to achieve.
Kristen Jellison

I am grateful that my fitness journey has led me to Form01945. Group exercise has been a passion of mine for many years. I have taught kickboxing, and small group training on the North Shore for the past 6 years. The energy and enthusiasm that is shared during a workout is empowering to everyone in the room, both physically and mentally. I strive to push people beyond mental road-blocks. Functional movement, strength and cardiovascular health are so important to longevity. My appreciation for fitness is balanced with my belief that a body needs to be fueled well, with seeking out healthful foods and hydration.
Lauren Kennedy

My goal as an instructor is to make a connection with each and every person who comes to my class. My hope is to give them confidence to stretch out of their comfort zone. I am a certified Personal Trainer and hold certifications in Barre Above and Extreme HITT Chaos training.

I bring a lifetime of active and living with over 30 years of fitness/personal training and dance coaching experience. I grew up in Marblehead and my fitness and athletic discipline started at a young age 6. I was a dancer, a competitive gymnast and a varsity soccer player at Marblehead High School.
Nicole Connolly
FORM
After working on a Bone Marrow Transplant ICU at Brigham and Women’s hospital I left the bedside nursing field to stay home and raise my two children. As an active mom, staying fit and prioritizing my health has always been extremely important to me.
